Monthly Membership Rates:

$150 per month for one child (base rate)

$260 per month for two children

$360 per month for three children

$450 per month for four children

$90 per month for each additional child beyond four

For 3 children, the total cost would be:​

Monthly membership: $150 + $110 + $100 = $360 per month

​*There is a one-time enrollment fee of $100 per child

What's Not Included?
At North Star Pediatrics, our Direct Primary Care (DPC) membership covers most of your child's everyday healthcare needs - but some services fall outside the membership.

Here's what's not included:

Emergency or Hospital Care

ER Visits

Hospital Admissions

Surgeries or Inpatient Care
*Tip: I recommend having a health insurance plan to help cover major unexpected events.

Specialist Visits

Care from specialists (e.g., cardiologists, neurologists, dermatologists)

Referrals are provided, but visits are billed separately by the specialist
​
Vaccines and Medications
-
Vaccinations and medications are not included by default
-
I can help coordinate vaccines through local health departments and pharmacies, or offer them at an additional cost, if available
-
I maintain a limited formulary and when possible will provide medications and vaccines at cost plus 10%
​
Advanced Labs & Imaging

Labs beyond basic in-office testing (e.g., blood work, X-rays, MRIs) are not included

I will help you access these services

Procedures Not Performed

Services requiring anesthesia, sedation, or surgery
